- Obtain/purchase white one-inch three ring binder with view windows for portfolio (should already have one from Fr/So year)
- Parent Permission Form Completed
- Signed Project Proposal Completed (one page only with signature line)
- Approved Project Supervisor
- Completed Letter of Recommendation Form
Due End of Second Quarter
- One Letter of Recommendation (Letter may be from a NPHS staff member or person not related to you)
- Research Paper Topic and Thesis Approved (in your English class)
Due End of Third Quarter
- Passing Research Paper Grade Earned _____________ (both paper and graded rubric must be in the portfolio)
- Five Hours of Community of Service (must be pre-approved)
- If hours are not completed or approved you will be required to complete 15 hours your Senior year
